An unannounced SHSI inspection was conducted on April 20th 2017 between 12:00pm and 2:00pm. There were 20 children in the direct care of 5 staff members. Children were observed preparing for nap, eating lunch, and watching a video. The operational responsibilities, the children's information, caregiver training, medication, nutrition, general record keeping, emergency drills, and transportation were observed and inspected. Standard #: VENDSUB-000-012-E Description: Based on record review, 1 of 10 staff members had not fully completed the preservice training. Evidence: Staff #1 had yet to complete the 10 hour required training. Plan of Correction: She is halfway through, she should finish by May.
Standard #: VENDSUB-000-031-A Description: Based on observation, the center does not all of the components of the emergency preparedness plan. Evidence: The center does not have a lock down procedure. Plan of Correction: Spoken with the owner, and something is already in writing and will be finalized and given to the center.
