Comments:
An unannounced Subsidy Health and Safety Inspection (SHSI) was conducted today. Observed 26 children directly supervised by 5 staff members. Ratios and supervision were in compliance. Operational responsibilities, children's information, caregiver training and development, parental involvement and notifications, nutrition and food services, general record keeping and reports, hazardous substances, emergency procedures, and transportation and field trips were reviewed. The children were engaged in various activities, to include: free play, and homework. Areas of non-compliance are identified in this report. The exit interview was conducted with the Site Director. Description: Based upon interview, a list of children?s allergies, sensitivities, and dietary restrictions was not posted. Evidence: There are three children who had been identified as having allergies and dietary restrictions; however, a list of these children?s allergies and dietary restrictions were not posted in the facility. The Site Director stated that the list is kept in a binder.

Description: Based upon documentation reviewed, children?s records were incomplete. Evidence: Two of the three children?s records reviewed did not contain a written allergy plan for diagnosed food allergies (Child A and Child B).
