An unannounced subsidy health and safety inspection was completed with the licensing inspection on 4/19/17. Thirteen staff records were reviewed. There was discussion with the director about the lock down procedures being revised to be specific for the center's location and also to include reunification plans. Staff #12 will need to complete the VA Pre-service training within 90 days of hire (hire date 1/20/17).
Standard #: VENDSUB-000-012-E Description: Based on record review, the center failed to ensure that two of 13 staff employed at the center had current first aid and CPR certification. Evidence: 1. Two staff who have been employed more than 30 days did not have current first aid and CPR certification. Both staff were certified March 2015 and the certification was valid for two years. The staff's certifications expired March 2017. Plan of Correction: One staff stated that the certifications have been updated but she has not received a copy of the certification card to show this. The director will get a copy of this for the staff person's file when it is received. If the other staff has not had updated training, the director will have the staff attend the training to update the certification. The director will monitor future expirations to ensure that certifications are current at all times.
