Standard #: VENDSUB-000-005-B Description: Based on a record review and interview, it was determined that the vendor failed to maintain, for each group of children, a written record (hard copy) of daily attendance that documents the arrival and departure of each child in care as it occurs. Evidence: 1. The center did not have written documentation of the arrival and departure times for all of the children in care. 2. Administrative staff confirmed that there was not documentation of the arrival and departure times for all of the children in care. Plan of Correction: The vendor responded: A plan will be developed with the teachers to ensure that the arrival and departure time for each child is recorded.
Standard #: VENDSUB-000-006-B Description: Based on a review of five children's records, it was determined that the vendor failed to ensure that there is a written care plan for each child with a diagnosed food allergy, to include instructions from a physician regarding the food which the child is allergic to and the steps to be taken in the event of a suspected or confirmed allergic reaction. Evidence: 1. The record for child #1 listed had documentation from the parent and the child's physician to indicate that the child was allergic to peanuts and eggs. However, there was no written allergy plan from a physician available for viewing during the inspection. 2. The record for child #2 listed had documentation from the parent and the child's physician to indicate that the child was allergic to fish. However, there was no written allergy plan from a physician available for viewing during the inspection. 3. Staff #1 (Program Director) reviewed the records fro child #1 and child #2 and confirmed that there was no a written allergy plan from a physician available for viewing during the inspection. Plan of Correction: The vendor responded: The parents will be asked to obtain a written allergy plan from a physician.
