VENDSUB-000 SPECIAL CARE PROVISIONS AND EMERGENCIES

Comments:
An unannounced Subsidy Health and Safety Inspection was conducted from 10am through 2:30pm. There were 50 children in care ages 16 months through ten years with eight teachers. Children and staff records were reviewed. Children were observed in teacher and self-directed activities including but not limited to outside play, table top toys/activities, lunch, restroom routines and nap.

Description: Based on record review and interview the vendor failed to ensure that within the first 90 days of employment all staff shall complete Virginia Preservice Training for Child Care Staff. Evidence: 1 - The record for staff 3 does not contain documentation that the required Virginia Preservice Training for Child Care Staff was completed. a. The staff has a documented date of hire as 10/14/15. 2 - Staff stated that staff 3 had not completed the required training.

Plan of Correction: Center management have informed the staff that they have 2 weeks to complete the required training. If at the end of two weeks the training is not completed center management will revisit and review the staff's employment status.

Standard #: VENDSUB-000-031-B-1
Description: Based on review of procedures and interview the vendor failed to ensure that procedures to address reuniting children with parents or authorized person designated by the parent to pick up the child are written. Evidence: 1 - The facility does not have a plan for reuniting children with their parents after and emergency. 2 - Staff 1 and 2 confirm that they have not created a reunification plan.

Plan of Correction: Center management will create and implement a reunification plan for children in care.
