Technical Assistance:
Today we discussed documentation in children's records regarding health issues, meal service, cot spacing and emergency preparedness.

Comments:
An unannounced monitoring inspection was conducted today from 11:30am through 2:40pm. There were 69 children in care with 13 staff. Children range in age from infant through six years old. Children and staff records were reviewed. Children were observed in teacher and self directed activities including but not limited lunch, nap restroom routines and hand washing. Violations in the area so cot spacing, emergency preparedness and meal service were observed and appear on the violation notice.

Violations:
Standard #: 22VAC40-185-440-E
Description: Based on observation the center failed to ensure that there is at least 12 inches of space between occupied cots. Evidence: 1 - During nap time there were six children sleeping on cots that had less than 12 inches space between occupied cots. 2 - Staff 3 and 4 confirmed that the cots did not have 12 inches of space between the occupied cots.

Plan of Correction: Center management will remind and retrain all staff on the necessity to place cots so that when occupied there is 12 inches space between them.

Standard #: 22VAC40-185-550-A
Description: Based on review of procedures and interview the center failed to provide an emergency preparedness plan. Evidence: 1 - There was not an emergency preparedness plan available for review during the inspection. 2 - Staff 1 and 2 confirm that the written emergency preparedness plan was not available for review today.

Plan of Correction: Center management will locate or rewrite, implement and train all staff in the center emergency preparedness plan.

Standard #: 22VAC40-185-560-F
Description: Based on observation and interview the center failed to demonstrate that they follow the most recent, age-appropriate nutritional requirements of a recognized authority such as the Child and Adult Care Food Program of the United States Department of Agriculture (USDA). Evidence: 1 - Staff 5 stated that each child in four/five year old room was served 1/2 a chicken patty sandwich, 1/2 a pickle spear and 2 slices of apple. 2 - 1 whole pickle spear weighed 1. 1 ounce. 2 apple slices weighed 1.2 ounces. 3 - The food available in the kitchen was not enough for each child to be served the required portion of fruit or vegetable. 4 - Staff 1 and 7 confirmed that children were not served the required portions of food.

Plan of Correction: Center management will review and retrain center staff on the necessity to ensure that each receives the required serving of each food group and that there is enough food available for each child to be served seconds.
