22VAC40-191 Background Checks (22VAC40-191)

An unannounced monitoring inspection was completed on April 21, 2017. The purpose of this visit was to measure the facility?s compliance with the licensing standards. The following ratios were observed: Infants-2:6; Toddlers-2:8; Two year olds-2:11; Three year olds-1:9; Four year olds- 1:9 The children throughout the facility were engaged in activity. The infants were being fed, sleeping, and playing on the floor. Children in toddler through four year old classes were observed playing with toys and riding tricycles outside. Staff was observed interacting with the children and providing assistance when needed. All areas of the center including classrooms, hallways, bathrooms and playgrounds were inspected. The facility was equipped with manipulatives and supplies and items were available to the children. The following information was reviewed: fire and shelter-in-place drills, injury prevention plan, liability insurance policy, parent agreements, and staff training information. Medication is administered and staff are MAT trained. Staff is certified in cpr/first aid and have completed the pre-service training. Last documented health inspection: 2/14/17 Last documented fire inspection: 3/2/17 During the inspection, five children's records and two staff records were reviewed. Approximate time on site: 10:00am to 01:45pm

Description: Based on a review of a sample of children's records, the facility did not ensure that documentation of a child's physical examination was in the child's record. EVIDENCE: The record of child # 4, start date 10/9/15, did not contain documentation of a physical exam.

Plan of Correction: The child's mother will bring the physical today.

Standard #: 22VAC40-185-60-A
Description: Based on a review of a sample of children's records, the facility did not ensure that a child's record contained all required items. EVIDENCE: The record of child #4, start date 10/19/15, did not contain proof of the child's identity.

Plan of Correction: The child's mother will bring in the birth certificate today.

Standard #: 22VAC40-185-270-A
Description: Based on observations at the facility, the facility did not ensure that all areas inside of the facility were clean. EVIDENCE: The inspector observed toilets in the pre-k room that were soiled and a toilet in the Discovery Preschool room that was visibly dirty on the seat and at the base of the toilet.

Plan of Correction: We will review daily cleaning procedures with the teachers.

Standard #: 22VAC40-185-440-L
Description: Based on observations at the facility, the facility did not ensure that pillows were not used by children under two years while sleeping. EVIDENCE: The inspector observed an infant, 10 weeks old, sleeping on her back on top of a large pillow designed for infant seating.

Plan of Correction: We will review safe sleep practices with the staff. Staff will be reminded to ask for assistance if needed. Infants are only permitted to sleep in cribs.
