The Licensing Inspector for the YMCA Afterschool Care @ Mt. Olivet conducted an unannounced SHSI monitor inspection on 4/25/2017. Upon arrival at 2:40 PM there were a total of thirty (30) school age children under the direct care & supervision of three (3) staff members. Three (3) children records, two (2) recently hired staff records, one (1) additional staff record & two (2) medication records were reviewed during today?s inspection. LI Anderson observed children during arrival/departure procedures, bathroom/hand washing procedures, snack/meal served, group activities, self-initiated activities and homework/quiet time. The exit interview including reviewing the Risk Assessment and areas of non-compliance found during today?s inspection on 1/17/2017 were reviewed with the Site Director on 1/17/2017. Inspection concluded at 5:30 PM. Standard #: VENDSUB-000-005-B Description: Based on inspection & record review, the Vendor failed to maintain a written record (hard copy) of daily attendance that documents the arrival and departure of each child in care as it occurs. EVIDENCE: The After school program did not have written documentation of daily attendance with sign-in/sign-out times. Plan of Correction: Staff will document sign-in/sign-out times using the new forms.
Standard #: VENDSUB-000-006-B Description: Based on record review & discussion with the staff, the Vendor failed to have a written allergy care plan for each child with a diagnosed food allergy, to include instructions from a physician regarding the food to which the child is allergic to and steps to be taken in the event of a suspected or confirmed allergic reaction. EVIDENCE: Several names of enrolled children were identified on the allergy chart with various food allergies. After school program did not have a written allergy care plan for each child. Plan of Correction: Site Directors will get information from the parents by the end of the week.
