Comments:
The licensing inspector conducted an unannounced monitoring inspection Tuesday, April 25, 2017 from approximately 11:15am to 1:40pm. The following ratios were observed with a total of 21 children present during the inspection:
Infants -5 children with 2 staff
Toddlers - 4 children with 1 staff
Twos - 5 children with 1 staff
Threes & Fours - 7 children with 1 staff
School Age Children ? no children were present during the inspection

During the inspection, children and staff were observed participating in a variety of activities. The Infants were observed playing on the floor with toys, being held and fed and eating lunch. The Toddlers were coloring and eating lunch. The Twos were observed having books read to them. The Threes and Fours were building with blocks, washing hands and transitioning into lunch. Children bring their lunches from home. Later children transitioned into nap time. All areas of the facility were inspected except the playground due to inclement weather. The facility was equipped with toys and supplies and items were available to the children.

The following information was reviewed: injury reports, daily attendance, daily sheet, infant feeding schedules, practiced emergency drill logs, snack menu, liability insurance and staff training information. Required postings were on display in the center. A member of management stated there is no medication present at the center at this time. Transportation is not provided at this time.

Violations:
Standard #: 22VAC40-185-60-A
Description: Based on the review of four children's records, two records did not contain the required information. Evidence: During the inspection, the licensing inspector observed the record for Child #1 (enrollment date 09/21/15) and Child #4 (enrollment date 01/21/16) did not contain documentation of an annual record review. The record for Child #4 did not contain the physical address for the two emergency contacts listed in the record. The record for Child #4 did not contain written agreements between the parent and the center which included authorization for emergency medical care should an emergency occur when the parent cannot be located immediately unless the parent states in writing an objection to the provision of such care on religious or other grounds; a statement that the center will notify the parent when the child becomes ill and that the parent will arrange to have the child picked up as soon as possible if so requested by the center; and a statement that the parent will inform the center within 24 hours or the next business day after his child or any member of the immediate household has developed any reportable communicable disease.

Standard #: 22VAC40-185-70-A
Description: Based on review of three staff records, one record did not contain the required information. Evidence: During the inspection, the licensing inspector observed the record for Staff #1 (hire date 09/12/13) did not contain documentation of one of the two character references required to be obtained prior to staff beginning employment. The record for Staff #1 did not contain documentation of orientation being completed before Staff #1 assuming job responsibilities.

Plan of Correction: These items were corrected while the licensing inspector was on site.

Standard #: 22VAC40-185-270-A
Description: Based on observation, the facility did not ensure all equipment was maintained in a safe and operable condition. Evidence: During the inspection, the licensing inspector observed a small red plastic chair located in the Threes/Fours classroom that contained cracks in the seat creating a pinching hazard.

Plan of Correction: We discarded the chair during the inspection. It must have happened this morning because the chair has not been cracked before, but is now thrown out.
