An unannounced monitoring inspection was conducted today 4/19/2017. The inspection began at 8:00 A.M. and concluded at 12:45pm. There were 72 children present with 14 staff members and the Program Director's were also present during the inspection. During the the time of the inspection all eight classrooms were observed, infant care was observed, diapering was observed, hand washing was observed, morning activities, arrival into the program, breakfast, art activities were observed, lunch time was observed, and emergency supplies were viewed. Discussion with the program director about: physicals and immunization requirements and subsidy requirements . The plan of correction is due in the licensing office on Wednesday, April 26, 2017.
Standard #: 22VAC40-185-140-A Description: Based on review of five children's records, the center failed to ensure that all physicals were obtained within the 30 days from the start date. Evidence: 1. The inspector found that child # 2 started the program on 11/21/2015 and the physical examination was obtained on 1/9/2016. Plan of Correction: The PD will ensure that the child's physical will be obtained within 30 days from the start date.
Standard #: 22VAC40-185-270-A Description: Based on observation of the center, the center failed to ensure that all areas and equipment of the center, inside and outside was maintained in a clean, safe and operable condition. Evidence: 1. The inspector observed in pre-k's room that the waynes coating is loose in the block area this poses a hazard to the children in care. 2. The inspector observed in the infant room an active leak in the ceiling. The ceiling tile was changed during the inspection and the new tile had evidence of water. On the day of the inspection it was raining steadily. 3. In the two year old room, the freezer portion of a plastic kitchen was broken off and had a sharp piece of plastic that could hard the children in care. 4. In the toddler room close to the cabinet area, the rug is starting to fray and could pose a trip hazard to the toddlers in care. Plan of Correction: During the inspection the ceiling tile was replaced and a roofer was contacted. The kitchen was removed from the two year old classroom. The wayne coating in the prek room was reattached to the wall.
