Comments:
Licensing Inspector arrived at 10:00 AM and departed at 12:30 PM. At the time of the tour, there were 89 children, ages infant through school-age, in care with 14 staff. Children were observed participating in free play activities, playing outside and eating lunch. The sample size was nine staff and nine children's records. All medication and other center documents were reviewed. Violations were noted in the areas of administration, physical plant, special care provisions & emergencies.

Standard #: 22VAC40-185-60-A
Description: Based on a review of nine children's records, it was determined that the facility did not ensure that all elements were present in each child's record. Evidence: 1. The record for child #1 did not contain place of employment and address for the father. 2. The record for child #2 did not contain an employment telephone number for the mother. 3. The record for child #3 did not contain any information for the father. 4. The record for child #4 did not contain any information on the father's employment.

Plan of Correction: Center management will contact parents to obtain all required informations for children's records.

Standard #: 22VAC40-185-270-A
Description: Based on observation and inspection, it was determined that the facility did not ensure that all areas of the center inside and outside are maintained in a safe and operable condition. Evidence: 1. The lock on the door to the diaper changing table in the infant room was loose and did not secure. 2. On the toddler playground the following things were noted: the pegs on the edging are loose and pose a tripping hazard, ther are areas of peeling paint and rust on the music panel and the dirt is worn away by the concrete sidewalk which poses a tripping hazard as the children enter and leave the playground. 3. There are roots located on the back / preschool playground that pose a tripping hazard for the children.

Plan of Correction: Maintenance orders will be submitted to have all physical plant issues taken care of.

Standard #: 22VAC40-185-510-G
Description: Based on a review of medication, it was determined that the facility did not ensure that all medication dispensed by a pharmacy was in the original container. Evidence: There were two Pro-air inhalers in the medication box that were not in their original boxes with the prescription label attached.

Plan of Correction: Center management will contact parents to obtain prescription labels.
