Comments:
An unannounced monitoring inspection was conducted on April 24, 2017 from 1:20 PM to 3:10 PM. Four staff were present with twenty-one preschool age children. Children were observed during nap time. Licensing Inspector reviewed standards related to supervision, activities, equipment, supplies, nutrition, emergency evacuation and shelter-in-place drill documentation, fire and health inspection reports, injury reports, children's records, staff trainings, emergency supplies and required postings. Please complete the columns for ?description of action to be taken? and ?date to be corrected? for each violation cited on the violation notice, and then return a signed copy to the licensing office within 5 calendar days of receipt. Violations:
Standard #: 22VAC40-185-140-A
Description: Based on review, facility did not ensure 1 of 4 children's records has a physical within a month of beginning attendance. Evidence: On March 24, 2017, the record for child #1 (start date September 6, 2016) does not have a physical.

Plan of Correction: Site Supervisor will ensure that all children's records are complete including physical forms.

Standard #: 22VAC40-185-450-A
Description: Based on observation, facility did not ensure 7 of 21 children have a top and bottom cover during nap time. Evidence: On March 24, 2017, 6 children were observed napping without a bottom cover and 1 child did not have a top cover.

Plan of Correction: Site Supervisor wille nsure all participants have both a top and bottom cover during nap time.
