Comments:
The licensing inspector conducted an unannounced monitoring inspection Tuesday, April 25, 2017 from 11:10am to 2:15pm. The children were observed listening to a book, singing songs and eating lunch. Lunch consisted of turkey sandwich, green beans, applesauce and milk. The lunch menu was posted. All classrooms were inspected today. Due to rainy weather the playground and the facility?s van was not inspected. Four children?s records and two employee records were reviewed during this inspection. Medication is being administered and was reviewed. The center's first aid kit and emergency supplies were inspected. Last emergency drill: 04/20/2017 Last shelter-in-place drill: 03/2017 Total enrollment today: 51 Today, the following child to staff ratios were observed: Infants ?4:1 Toddlers ?7:2 Young Twos ? 7:1 Older Twos-8:1 Threes ?15:2 Pre-K ?10:1 School Age ? not present The violations from the previous inspections were checked for corrections. Standard #: 22VAC40-185-140-A
Description: Based on a review of children's records on 04/25/2017, the center did not ensure that two of four records reviewed contained a physical examination before the child's attendance; or within one month after attendance. EVIDENCE: 1. The physical in the record of Child #1, whose first day at the center was 08/29/2016, is dated 10/12/2016. 2. The physical in the record of Child #2, whose first day at the center was 01/23/2017, is dated 03/03/2017.

Plan of Correction: Going forward will require the physical at enrollment.

Standard #: 22VAC40-185-60-A
Description: Based on a review of children's records on 4/25/2017, the center did not ensure that, when applicable, one of four children's files reviewed contained a work number for the parent's place of employment. EVIDENCE: The record of Child # 2 did not have a phone number documented for the place of employment listed for the child's mother.

Plan of Correction: Will get information from parent today at pick up.

Standard #: 22VAC40-185-550-E
Description: Based on a review of documentation and staff interview on 04/25/2017, the center did not ensure that a record of the practice drills conducted were maintained. EVIDENCE: The record of drills reviewed did not have documentation on a evacuation drill for the month of March. Staff #3 stated that an evacuation drill was done that month; however it wasn't written down.

Plan of Correction: Will have two staff check to ensure drills are documented.
