22VAC40-191 Background Checks (22VAC40-191)

Thank you for your time and assistance during the unannounced monitoring inspection conducted on April 26, 2017, between the hours of approximately 10:12 a.m. and 12:50 p.m. Forty-nine (49) children were in the care of six staff members during the on site inspection. Three children's records were reviewed and four staff records were reviewed and/or updated. No medications were administered/on site, on this date. Safety equipment (first aid kits and flashlights), posted information (license, inspection documentation, weekly menu, emergency evacuation diagrams and plan, emergency telephone numbers and classroom schedules) and available documents (emergency evacuation drill/shelter-in-place drill records, injury report form and local agency inspection reports) were observed/reviewed. The morning snack included crackers, tomatoes and cucumbers and lunches were provided by the children's parents. The facility was observed to be clean and supplied with a variety of age/stage program specific learning and play materials. Program observation was completed. Activities provided on this date included: children's work cycle in the 3 through 6-year-old classrooms (alphabet letters used by children to make words and sentences, clay, math using counting beads, creating/gluing a map of the United States, puzzles, books and painting); group time in the toddler classroom ( poems, songs/movement and story); handwashing and restroom procedures; outdoor play; lunch; naptime and departure/pick-up of children. Staff/child interactions, behavior guidance and daily routines/transitions were observed. Staff members were observed to be positive, patient and supportive in their interactions with the children in their care. Refer to the Violation Notice for exceptions to compliance. Standard #: 22VAC40-185-540-C
Description: Based on observation of first aid kits and discussion with staff on April 26, 2017, the program failed to ensure an antiseptic cleansing solution/pads was included in the first aid kits.
1. Observation of the first aid kit on the first floor, on this date, revealed that an antiseptic cleansing solution/pads was not included with the first aid supplies. Staff #1 confirmed the lack of an antiseptic cleansing solution/pads in the first aid kit.
2. Observation of the first aid kit on the second floor, on this date, revealed that the antiseptic spray in the first aid kit had an expired date (3/03). Staff #3 confirmed the date on the antiseptic spray.
3. Staff members were not able to locate any additional antiseptic cleansing solution/pads in the facility, on this date.

Plan of Correction: Will get antiseptic pads/solution for both kits.

Standard #: 22VAC40-191-40-D-1-C
Description: Based on review and/or update of four staff records and discussion with staff on April 26, 2017, the program failed to obtain an updated criminal history record clearance for one employee and an updated central registry clearance for one employee, before three years since the dates of the last clearances. Evidence: 1. Review of staff #2's record, on this date, determined that an updated criminal history record clearance was not on file (date of the last clearance - 4/18/2014). 2. Review of staff #4's record, on this date, determined that an updated central registry clearance was not on file (date of the last clearance - 10/02/2013). 3. Staff #1 and staff #6 confirmed the lack of updated clearances.

Plan of Correction: Will send in both forms.
