Licensing Inspector conducted a Subsidy Health and Safety Inspection from approximately 11:45am to 4:45pm. At the time of the tour there were 21 children, ages four months to four years old with five staff. Children were observed napping, during tummy time and playing freely. The sample size was eight staff records and four children's records.
Standard #: VENDSUB-000-004-F Description: Based on observation and inspection of the facility, the vendor did not post, with parental approval, or keep in a way that is immediately accessible to staff, a current list of all children?s allergies, sensitivities, and dietary restrictions. Evidence: The center had not posted an allergy indicated in the record for Child #1 and the center had not obtained parental permission to post an allergy of Child #4. Plan of Correction: We will notify parents to write a letter stating it is ok to have the allergies posted for Child #4. I will put Child #1 on the allergy list and notify the parent.
Standard #: VENDSUB-000-005-B Description: Based on review, the vendor did not maintain a written record (hard copy) of daily attendance that documents the arrival and departure of each child in care as it occurs. Evidence: The center did not include documentation of the arrival of seven school age children on the date of the inspection. Plan of Correction: The log will be closely monitored to ensure that it does not happen again.
Standard #: VENDSUB-000-012-B Description: Based on record review, in one of eight staff records reviewed, the vendor did not ensure that all staff shall complete Virginia Preservice Training for Child Care Staff. Evidence: There was no documentation of the Virginia Preservice Training for Staff #8. Plan of Correction: The employee will receive the training.
Standard #: VENDSUB-000-012-E Description: Based on review, the vendor did not ensure that current all staff providing care to children shall have current certification in cardiopulmonary resuscitation (CPR) appropriate to the age of children in care. The training shall include an in-person competency demonstration and current certification in first aid. Evidence: The center had documentation of an online CPR and first aid testing certification but it did not include documentation of an in-person competency demonstration for Staff #1, Staff #2, Staff #4, Staff #7 and Staff #8. Plan of Correction: Each employee does have up to date training but it was not the accepted version. We will work hard to get everyone with the correct and accepted training. Will contact trainer tomorrow to set up training for those staff persons.
Standard #: VENDSUB-000-031-B-3 Description: Based on review, the vendor did not have lockdown procedures, to include facility containment where included in the emergency preparedness plan. Evidence: The emergency preparedness plan did not include all procedural components for lockdown procedures. Plan of Correction: All of the missing components for the lockdown procedures will be added to the emergency plan.
