An unannounced monitoring inspection was conducted on April 28, 2017. At arrival to the facility, four staff were present with 21 children. Additional staff were present for support and administrative purposes. Children were observed listening to a story, during toileting and handwashing, playinig freely and having lunch. Three children's records and one staff record were reviewed on this date. Licensing Inspector reviewed supervision, activities, equipment, hand washing procedures, nutrition, documentation, children and staff records, emergency evacuation and shelter-in-place drill documentation, fire and health inspection reports, liability insurance coverage, emergency supplies and required postings. There are currently no medications being administered. Violations were reviewed with and verified by the Program Director on this date. The outdoor playground equipment, to include the see-saw, is not currently being utilized.
Standard #: 22VAC40-185-60-A Description: Based on record review, in three of three children's records reviewed, the licensee did not ensure that each child's record was maintained with all required information. Evidence: 1. During the inspection conducted on April 28, 2017, all three children's record did not include the name of any additional programs or schools that the child is concurrently attending and the grade or class level. 2. The record for Child #1 did not include the work phone number for one parent and the names of any previous child day care and schools attended by the child. 3. The record for Child #3 did not include a complete address for one emergency contact and the names of any previous child day care and schools attended by the child. Plan of Correction: Program Director corrected the record for Child #1. The parents will be contacted to make any necessary corrections.
Standard #: 22VAC40-185-260-A Description: Based on review, the licensee did not provide to the licensing representative an annual fire inspection report from the appropriate fire official having jurisdiction. Evidence: During the inspection conducted on April 28, 2017, the most recent fire inspection report for the facility had expired and was dated 3/25/16. Plan of Correction: Program Director will contact the Fire Marshall's Office.
Standard #: 22VAC40-185-280-B Description: Based on observation and inspection of the facility, the licensee did not ensure that hazardous substances such as cleaning materials, insecticides, and pesticides shall be kept in a locked place using a safe locking method that prevents access by children. Evidence: 1. During the inspection conducted on April 28, 2017, the licensing inspector observed the following in an unlocked closet in the Infant room: seven cans of shaving cream and three cans of spray paint that were labeled keep out of reach of children and warning or danger. 2. In the Two Year Old classroom there was a bottle of hand sanitizer on the counter in the back of the classroom that was labeled keep out of reach of children and warning. Plan of Correction: Program Director spoke with staff. All items have been locked.
Standard #: 22VAC40-185-500-B Description: Based on observation and inspection of the facility, the licensee did not ensure that a nonabsorbent surface for diapering or changing shall be used. For children younger than three years, this surface shall be a changing table or countertop designated for changing and that the diapering surface shall be used only for diapering or cleaning children. Evidence: 1. During the inspection conducted on April 28, 2017, the licensing inspector observed a two year old child being helped into a pull-up while standing in the bathroom. A changing table or countertop was not used for changing the child. 2. There was a box of gloves on the changing pad in the Two Year Old Classroom. Plan of Correction: Gloves were removed during the inspection. A changing table will be used for children younger than three years old.
Standard #: 22VAC40-185-560-F Description: Based on observation and inspection of the facility, the licensee did not ensure that when centers choose to provide meals or snacks, the following shall apply: Centers shall follow the most recent, age-appropriate nutritional requirements of a recognized authority such as the Child and Adult Care Food Program of the United States Department of Agriculture (USDA) and A menu listing foods to be served for meals and snacks during the current one-week period shall: be dated and indicate substituted food. Evidence: 1. During the inspection conducted on April 28, 2017, the licensing inspector observed children between the ages of three to five years old having lunch. The children were served a 1/2 cup of milk where they should have been served 3/4 cup. The children were served less than a 1/2 cup of corn and less than a 1/2 cup of applesauce where they should have have been served 1/2 cup of two fruits and/or vegetables. 2. The menu posted in the center was not dated and did not indicate substituted food for the date of the inspection . Plan of Correction: We will use a measuring cup or spoon while serving. The menu was corrected during the inspection.
Standard #: 22VAC40-185-560-N Description: Based on observation and inspection of the facility, the staff members did not ensure that no child shall be allowed to drink or eat while walking around. Evidence: During the inspection conducted on April 28, 2017, the licensing inspector observed a child walking around and drinking milk in the Four Year Old Classroom. Plan of Correction: We will make sure children sit while eating and drinking.
