Comments:
An annual shsi inspection was conducted today, this inspection was delayed because the vendor was away from the home when the first two inspections were attempted. The vendor has 4 children care at this time a total of 6 points.
She has space for the children to play and lots supplies The children were napping at the beginning of the inspection. The children who woke up was observed playing. Today's breakfast was eggs and hot dogs and bread, Lunch today was chicken and rice and bananas This inspection was started at 2:00 and ended at 4:00. Standard #: 22VAC40-665-170-B
Description: Based on the review of the children's records the vendor failed to get some of the information needed to make the children's records complete.
Evidence:Child A,B,C,D and E are all missing emergency contacts that are not the parents, they are also missing documentation of proof of ID. Child E only has a physical in his files but are missing all the other documentation.

Plan of Correction: I will ask the parents for proof of ID and emergency contacts and I will get Gabriel file from infant toddler.

Standard #: 22VAC40-665-410-B
Description: Based on the review of the vendors emergency drills, the vendor failed to record some of the emergency drills conducted during the last year.
Evidence: The shelter in place drills and lock down drill was not recorded in the last year. According to the vendor she conducted the drills but did not record them.

Plan of Correction: I will make sure I record the drills for this next year.
