22VAC40-191 Background Checks (22VAC40-191)

An unannounced full renewal inspection was conducted today. There were 61 children in care with a total of 11 staff members assigned to their direct supervision. The children were observed playing outside, reading books, practicing phonics skills, sorting animals, and eating lunch. The children were served cheeseburgers, mixed vegetables, and milk for lunch today. This inspection was completed between approximately 10:00 am and 12:30 pm. Thank you for meeting with me today. Standard #: 22VAC40-185-60-A
Description: Based on a review of children's records, it was determined that one record was not complete. Evidence: 1. Child record #4 does not include a physical exam report and shot record.

Plan of Correction: The parents will be asked for the missing information.

Standard #: 22VAC40-185-240-C
Description: Based on a review of training logs, it was determined that two staff did not attend at least 16 hours of training during the past year. Evidence: There is no documentation to show that staff A and staff B attended any training courses during the past year.

Plan of Correction: Training will be completed and fully documented this year.

Standard #: 22VAC40-185-260-A
Description: Based on a review of administrative documents, it was determined that the fire inspection permit has expired. Evidence: The annual fire inspection permit expired on 3/31/17.

Plan of Correction: I will follow-up with the county to have the fire permit renewed.

Standard #: 22VAC40-185-270-A
Description: Based on observations, it was determined that two areas of the center have not been maintained as required. Evidence: 1. In the Toddler classroom, the plastic covering on the side wall of the children's sink is peeling leaving an area of exposed particle board. 2. There is one wheel missing from the stack of cots in the Pre-K A classroom, causing the stack of cots to be unstable.

Plan of Correction: I have already requested the sink cover to be replaced. I will follow-up with maintenance to make sure it gets replaced. I will ask maintenance to replace the missing wheel.

Standard #: 22VAC40-185-280-B
Description: Based on observations, it was determined that potentially hazardous substances were not locked. Evidence: 1. In the Infant B classroom, cleaning products were stored inside two changing table cabinets. These cabinet doors were not locked. 2. The laundry room is located inside the 2's classroom. The door to this room was not locked. Various cleaning products, detergents, and soap are stored inside this room.

Plan of Correction: The changing table cabinet doors were locked during the inspection. The laundry room door will be locked.

Standard #: 22VAC40-185-330-B
Description: Based on observations, it was determined that there is not enough resilient surfacing on the playgrounds. Evidence: There is not enough mulch at the bottom of each slide on both playgrounds. In these areas, the mulch has worn down to a level of dirt.

Plan of Correction: Mulch will be ordered.

Description: Based on observations, it was determined that one staff person was not seated with the children as required. Evidence: During this inspection, the 8 children in the Pre-K A classroom were served lunch. The staff person assigned to this class of children did not sit with the children during lunch.

Plan of Correction: All staff will be reminded to sit with the children during lunch.

Standard #: 22VAC40-191-40-D-1-B
Description: Based on a review of staff background checks, it was determined that one staff person has not signed a Sworn Disclosure Statement (SDS.) Evidence: There is no documentation of a signed SDS for staff B, who began employment on 3/30/16.

Plan of Correction: I will ask her to sign the form today.
