22VAC40-191 Background Checks (22VAC40-191)
An unannounced monitoring inspection was conducted from 9:00am to 11:30am Upon arrival there were 36 children in care with 6 staff present. A sample size of 2 staff and 4 children records were reviewed. The center did not have any medication. Children were observed during circle time, practicing colors, doing table toys and on the playground.
Standard #: 22VAC40-185-70-A Description: Based on a record review the center failed to ensure all required information was kept for each staff person. Evidence: Staff #1 (doh 07/07/15) did not have written documentation that she met the qualifications of her position as program leader. Staff #1 was observed acting in the role of program leader during the inspection. Plan of Correction: Staff #1 has a CNA. The director will obtain the documentation from staff #1.
Standard #: 22VAC40-185-270-A Description: Based on observation the center failed to ensure areas and equipment of the center, inside and outside were maintained in a safe condition. Evidence: The bottom of the fence between the two preschool playgrounds was rusted. Plan of Correction: The director will call the fence company and have the rust removed from the fencing.
