Technical Assistance:
Inspector spoke with the Director about 22VAC40-185-60 during this inspection.

Comments:
An unannounced monitoring visit and facility tour was conducted from 2:15 pm- 3:45 pm. At the time of inspectors' arrival there were 38 pre-school and school aged children in care with 4 staff members and 1 administrator. Children were observed participating in indoor play, nap, snack, transitioning between activities, and hand washing. A sample of 5 children's records and 4 staff on-site records were reviewed. Injury reports were also reviewed. Areas of noncompliance are identified on the violation notice and were discussed with the director in an exit meeting at the conclusion of this inspection. An additional record review of 4 staff members was conducted at the administrative office due to staff records being kept off-site.

Standard #: 22VAC40-185-60-A
Description: Based on record review and interview it was determined the center did not ensure each child's record contained all of the required components. Evidence: The records for child #2 and child #5 did not contain the complete address for one of the two required emergency contacts. Both children were in care and the Director could not locate this information during this inspection.

Plan of Correction: Director will request this information from the parents at pick-up.

Standard #: 22VAC40-185-560-G
Description: Based on observation the center did not ensure that when food is brought from home the food container is clearly dated. Evidence: 12 of the 25 lunch boxes observed were not dated correctly.

Plan of Correction: Director will retrain staff on dating and labeling food brought from home.
