Discussed with administration the importance of a behavior plan for children when a child's behavior begins to effect the daily program, students, and staff in the classroom.
An unannounced monitoring inspection was conducted from 10:00 am - 1:40 pm. There were 83 children in care being supervised by 14 staff members. The children were observed participating in outdoor play, structured center play, stories and singing, hand washing and diapering were observed. Lunch served today is fish sandwich on wheat bun, mashed potatoes, pineapple, and milk. Ten children's records were reviewed and in compliance. Ten staff records were reviewed. Four medications were reviewed and in compliance. Medication Administration Training certificates were reviewed. All staff on site are current in CPR and First Aid Training. Standard #: 22VAC40-185-160-C Description: Based on review of ten staff records it was determined that not all TB (tuberculosis) screenings were repeated every two years. Evidence: The most recent TB screening on file for Staff A was dated 09/17/2014. Plan of Correction: Management will monitor and maintain staff TB screenings to ensure that each staff member has a current TB test. Management will check staff files monthly to make certain that all necessary job screenings are up to date and are never allowed to expire. The referenced teacher has completed her TB test.
Standard #: 22VAC40-185-270-A Description: Based on indoor and outdoor observation of the center it was determined that not all areas of the center were maintained in a safe and operable condition. Evidence: 1. In the two's room there is a crack on the top portion of the changing table with sharp edges exposed and accessible to children while being diapered on the changing table. 2. In the Toddler 1 room on the bottom cabinet below the sink there is a piece of trim that has broken off leaving sharp edges and press board exposed at child level and accessible to children. 3. There is a light fixture hanging down from the overhang outside at the exit to the infant/toddler playground. 4. There is a piece of trim hanging with sharp edges exposed on the fish tank cart next to the bottom left wheel in the Private Kindergarten room accessible to children. Plan of Correction: Management will complete daily safety checks to ensure that the center is always in a safe and operable condition. A facilities technician will visit the center within the next 14 days to replace the changing table in the Two?s class, remove and replace the trim in the toddler room on the bottom of the cabinet sink, remove the damaged light fixture outside the infant/toddler program and replace, lastly the technician will repair the fish tank cart that is located in the Private Kindergarten room.
Standard #: 22VAC40-185-350-E-4 Description: Based on observation of the Private Kindergarten room it was determined that not all ratios of 1 staff member for every 10 children for children under the age of five were not followed. Evidence: There were twelve children being supervised by one staff member in the Private Kindergarten classroom which would normally be a 1:18 ratio however, three children from the Pre-K classroom (Child D, Child E, Child F) were moved to the Private Kindergarten class lowering the required ratio to 1:10. Plan of Correction: Management will ensure child to staff ratios are in compliance during all hours of operation. Child to staff ratios will be monitored throughout the day by completing 15 minute checks, and adjusting staff schedules as necessary so that reassignment of children is never required. We also have plans to onboard three new employees within the next 45 days.
Standard #: 22VAC40-185-350-F Description: Based on observation and interview with staff, it was determined that the center was reassigning children from their regular classroom for reasons other than administrative necessity disrupting the children's schedule and attachment to their regular group. Evidence: Three children (Child A, Child B, Child C) were reassigned from their normal class in the three's classroom to another three's classroom that had two staff members assigned to maintain ratio. Three children (Child D, Child E, and Child F) were reassigned from the Pre-K classroom to the Private Kindergarten classroom to maintain ratio. Moving these children made the Private Kindergarten classroom out of ratio as the 1:18 ratio no longer applied with the 4 and 5 year old children included. Plan of Correction: Management will ensure child to staff ratios are in compliance during all hours of operation. Child to staff ratios will be monitored throughout the day by completing 15 minute checks, and adjusting staff schedules as necessary so that reassignment of children is never required. We also have plans to onboard three new employees within the next 45 days.
