Technical Assistance:
Discussed the requirements for the subsidy visits with the director and District Director.

Comments:
An unannounced monitoring inspection was conducted from 9:00 am-1:30. At the time of entrance, 122 children were in care with 21 staff members present. Children were observed engaged in group activities, blocks, puzzles, coloring, planting seeds and outdoor play time. The playground and surfacing was inspected. A selection of staff and children records, medications and other documents were reviewed. Background checks of new staff since last inspection were reviewed. Violations:
Standard #: 22VAC40-185-510-G
Description: Based on medication review, the center does not have a prescription label that is in accordance to the prescribers instruction pertaining to dosage. Evidence: Child #1 medication form completed by the physician says to administer .15 mg of the medication. The medication and prescription label on the medication is 0.3 mg dosage.

Plan of Correction: Parents have been given updated paperwork and doctor is completing and signing off to ensure dosage is accurate.
