Comments:
An annual subsidy health and safety inspection (SHSI) was conducted from 6:35 a.m. to 7:45 a.m. with the director and one staff. Three staff records were reviewed. SHSI requirements were reviewed. Description: Based on review of documentation, a child will a diagnosed food allergy did not have a written allergy plan. Evidence: Child #1 has a shellfish/tree nut allergy and did not have a written allergy care plan, to include instructions from a physician regarding the food to which the child is allergic to and steps to be taken in the event of a suspected or confirmed allergic reaction.

Plan of Correction: This information will be requested and obtained from the parent.
