Comments:
An unannounced Subsidy Health and Safety Inspection was conducted on May 1, 2017 from approximately 3:00 p.m. until 6:00 p.m Upon entrance there were 92 children and 6 staff present with children. The record review sample consisted of 6 children's records and 6 staff records. Children were observed eating snack, working on homework, playing board games and video games. The results of the inspection were reviewed with the center director.

Violations:
Standard #: VENDSUB-000-033-F
Description: Based on staff interviews, the vendor did not ensure that staff who prepare and serve food to children, or supervise meals, were aware of the food allergies, sensitivities, and dietary restrictions for each child. Evidence: During the inspection on May 1, 2017, when interviewed, all of the six center staff were not aware of all of the children and their food allergies and dietary restrictions/ sensitivities. This was verified with the center director.

Plan of Correction: The center supervisor will ensure staff who prepare and serve food are aware of any food allergies, sensitivities and/or dietary restrictions.

Standard #: VENDSUB-000-004-F
Description: Based on staff interviews, the vendor did not ensure the staff posted, with parental approval, or kept in a way that is immediately accessible to staff, a current list of all children?s allergies, sensitivities, and dietary restrictions. Evidence: During the inspection on May 1, 2017, the list of with children's allergies was not posted in a place or kept in a place immediately accessible to staff. This was verified with the center director.

Plan of Correction: The center supervisor will ensure a current list of children's allergies, sensitivities and dietary restrictions are posted.

Standard #: VENDSUB-000-006-B
Description: Based on observation, the vendor did not ensure there was a written allergy care plan for each child with a diagnosed food allergy, to include instructions from a physician regarding the food to which the child is allergic to and steps to be taken in the event of a suspected or confirmed allergic reaction. Evidence: During the inspection on May 1, 2017, there was one child (child #2) in the center who was identified has having food allergies (nuts and eggs) and there is not documentation to include a physician's information on how to address an allergic reaction. This was verified with the center director.

Plan of Correction: The center supervisor will ensure that physicians instructions for allergic reactions are included in the records of children who have allergies.

Standard #: VENDSUB-000-012-F
Description: Based on observation, the vendor did not ensure staff employed by an approved vendor prior to the effective date of the vendor agreement completed CPR and first aid training as required by this section within 90 days of the effective date of this agreement. During this 90 day period, there must always be at least one staff with current CPR and first aid training present during operating hours of the center. Evidence: During the inspection on May 1, 2017, the records for six staff were reviewed and four (staff #1, #2, #5 and #6) did not have documentation indicating they had obtained the CPR and first aid training . The center supervisor verified this during the inspection.

Plan of Correction: Newport News PR&T maintains CPR& first aid certifications of all full time staff. Administration will continue to work toward certification of all other staff.

Standard #: VENDSUB-000-031-B-1
Description: Based on observation, the vendor did not ensure there were procedures to address reuniting children with parents or authorized person designated by the parent to pick up the child during an evacuation. Evidence: During the inspection on May 1, 2017, there were not reunification plans documented . This was verified with the center director.

Plan of Correction: The center supervisor will ensure that reunification plans are included in the emergency evacuation plans.
