Technical Assistance:
Discussed the upcoming renovations to the building including; flooring, trim, and new paint. Discussed the capacity changes to the classrooms as some rooms are now being used for different age groups.

Comments:
An unannounced monitoring inspection was conducted today from 9:00am-11:15am. There were 86 children (infants-5yrs.) directly supervised by 15 staff. Children were observed participating in "Show and Share", group play, circle time activities, singing songs, and infants were observed having tummy time. The physical plant, 5 staff records, 5 children?s records, medications, evacuation drills, injury reports, emergency supplies, and policies were inspected. Diapering and hand-washing procedures were also observed. There was an abundant supply of materials for the children. Areas of non-compliance are identified in the Violation Notice. Violations:
Standard #: 22VAC40-185-270-A
Description: Based on observation, areas and equipment inside of the center were not maintained in a clean, safe and operable condition. Evidence: 1. In the Beginners, Intermediate, Pre-Kindergarten 1 and 2 classrooms, the tile and trim in the bathrooms were heavily worn, warped and cracking. The metal divider walls in the Beginner and Intermediate bathrooms were beginning to rust and chip at the bottom. 2. In the Beginners classroom, the wall behind the changing table had a large hole and the trim was beginning to pull away from the wall.

Plan of Correction: Repairs are scheduled for the flooring and tile. A work order has been placed for the other repairs.

Standard #: 22VAC40-185-510-N
Description: Based on observation, when an authorization for medication expired, the parent was not notified the authorization needed to be renewed. Evidence: In the Intermediate classroom, a medication was found with an authorization that expired in December 2016. (Child #1)

Plan of Correction: The parent has been asked to bring in a new authorization form.

Standard #: 22VAC40-185-520-C
Description: Based on observation, diaper ointments and creams in 1 of 7 classrooms were not stored in a location inaccessible to children. Evidence: In the Beginners classroom, a container of diaper cream was found in the cabinet under the changing area. The cabinet was not locked.

Plan of Correction: Corrected at the time of inspection. The cabinet was locked by a staff member.
