Comments:
Thank you for your time and assistance with the unannounced monitoring inspection conducted today, May 5, 2017. The on-site inspection was from 2:30 p.m. to 6:05 p.m. On this date, there were 141 school-aged children in five groups and in the care of 12 staff members. Two extra staff members were available to assist if needed. The total enrollment for this session's program was 196 children. The records of two staff members' were reviewed and three staff members' records were checked for updates. There were five children's records reviewed and the medication and medication paperwork were reviewed for four children. There were eight staff members and five children interviewed. The exception to compliance was noted on the violation notice and the risk rating was noted on the supplemental page. Items reviewed/observed during the visit included: required posted information, first aid kit, emergency supplies, snack menu, arrival procedures, transitions, outdoor play, large motor games in the Arena, hand washing and bathroom procedures, snack, games, and dismissal procedures. Standard #: 22VAC40-185-130-A
Description: Based on review of children's records and verification from the director, the center failed to obtain documentation of a child's immunizations before the child's first date of attendance. Evidence: 1. The record of Child # 4 contained an immunization record that was dated 11/02/2016 but the child's first date of attendance what 10/14/2016. 2. The director verified the immunization record was obtained after the child enrolled.

Plan of Correction: In the future, no child will be allowed to attend the program without an immunization record on file.
