22VAC40-191 Background Checks (22VAC40-191)

An unannounced monitoring inspection was conducted today, May 8, 2017 as a follow-up to the issuance of the conditional license. The Inspector initiated the inspection at 10:30 am and concluded the inspection at 12:15. Additional information for staff records was reviewed at the CDI Head Start Administrative office from 12:45 pm until 1:40 pm. Upon the inspector's arrival there were 15 children in care with three staff. Children were just finishing center time and observed gathering on the carpet for music and movement. Children stretched and counted to one hundred along with a music cd. Children also marched around the room and acted out the actions for "going on a bear hunt". Staff read a story to the children and they discussed the parts of a book. Bathroom breaks and hand washing was completed before children and staff went to the school cafeteria for lunch. Today's lunch consisted of hot dog on a whole grain bun, baked beans, pears and milk. Later children had time on the playground. End of year student evaluations were being conducted today. Staff worked with children one on one reviewing concepts and alphabet. The classroom is equipped with age and stage appropriate materials that are in operable condition. There is a parent board with the license, last inspection notice, emergency numbers, daily schedule and emergency evacuation procedures posted. The facility's first aid kit and non-medical emergency supplies were inspected and found complete. The inspector interacted with staff and children in care. Three staff records and three children's records were reviewed. Staff have current CPR, first aid, daily health observation and MAT. Medications are administered by the public school nurse. The most recent evacuation drill was documented on 04/21/2017. One of the two shelter-in-place drills was conducted on 03/21/2017. A second shelter-in-place drill will need to be completed before the end of the school year. Health Inspection 03/15/2017 Fire Inspection 05/09/2016 ( a fire inspection is scheduled for 05/10/2017) The violation cited at the previous inspection on December 7, 2016 were checked for corrections. Description: Based on review of staff records, the facility did not obtain an original criminal history report within 30 days of employment. Evidence: 1. Staff #2, with a hire date of 03/28/2017, did not have documentation of an completed original criminal history report on file. 2. Staff #3, with a hire date of 02/27/2017, had documentation of the completed original criminal history record, however it was dated 04/20/2017, exceeding the 30 day allowance from hire to obtain.

Plan of Correction: Per the Administrator: We will continue to send the request off at the time of hire and if they are not received within 30 days, we will call the agency and request the status of the background checks.
