An unannounced monitoring visit and facility tour was conducted from 2:40 pm-4:35 pm. At the time of inspectors' arrival there were 75 preschool aged children in care with 10 staff members and 1 administrator. Children were observed participating in instructional time in the classroom, transitioning between activities and toileting and hand washing. A sample of 5 children's records and 5 staff records were reviewed. Medications and injury reports were also reviewed. Areas of noncompliance were discussed with the center administration at the conclusion of this inspection and are detailed on the violation notice.
Standard #: 22VAC40-185-270-A Description: Based on observation it wad determined the center did not ensure that all areas and equipment of the center, inside and outside, were maintained in a clean, safe and operable condition. Evidence: 1. There was peeling paint and splintering wood on the outdoor stationary play equipment presenting potential injury hazards. 2. There was an open box of gallon ziplock bags in an unlocked floor level cabinet below the sink in the toddler classroom presenting potential suffocation/choking hazards. Plan of Correction: Center will repair outdoor stationary play equipment by 5/21/17. Center will ensure plastic bags inaccessible to children in care 5/10/17.
Standard #: 22VAC40-185-280-B Description: Based on observation it was determined the center did not ensure all hazardous substances were kept in a locked place using a safe locking method that prevents access by children. Evidence: There was a spray bottle of sanitizing spray in an unlocked floor level cabinet below the sink in the toddler classroom presenting potential suffocation/choking hazards. Plan of Correction: Center will ensure all chemicals are kept locked.
Standard #: 22VAC40-185-550-D Description: Based on record review it was determined the center did not ensure that evacuation drills were practiced monthly. Evidence: There was no evacuation drill practiced for the month of April 2017. Plan of Correction: Center will ensure that evacuation drills are practiced monthly.
