05/10/2017 Arrived 10:50am Departed 12:30pm An unannounced monitoring inspection was conducted today. There were 15 children in care with three staff supervising at all times. Children were observed working on a craft, playing in centers, having group time and eating lunch. Six children's records were reviewed. Violations from the last inspection were reviewed. Please call your inspector at 276-698-9981 with any questions.

Description: Based on the review of six children's records the facility did not complete the enrollment information for each child. Evidence: 1. The records for child 1-6 do not contain any information on chronic health conditions.

Plan of Correction: The director will get the information from the parents and add it to the forms.
