22VAC40-191 Background Checks (22VAC40-191)
An unannounced Renewal inspection was conducted in the Center on May 11, 2017. Upon arrival in the Center, the LI verified 89 children were present. The children were grouped in four classrooms. Each classroom was staffed in staff/child ratio. The children were observed in their classrooms during free play. Hand-washing procedures were observed before lunch. The lunch meal was observed in one classroom. After lunch the children were observed during nap/rest time. A sample of nine complete records was reviewed, 5 staff and 4 children. Documentation of the Center's annual fire and health inspections was current. Medications for 5 children were reviewed. Areas of the Center inside and outside were maintained in a clean condition. Dismissal procedures were observed at the end of the program day. A discussion was held with the Owner regarding over-the-counter medications. Findings of the inspection were reviewed with the Owner/Licensee at the exit. The inspection was conducted from approximately 10:15am to 5:00pm. If you have any questions, please call (540)309-2310. Standard #: 22VAC40-185-510-L Description: Based on a review of medications, documentation of a child's medication record and staff report, the Center failed to document a medication error. Evidence: The LI reviewed medications for child a. The prescribed medication was to be administered each day at 2:30pm. The medication record did not indicate the medication was administered on May 8 and May 10, 2017. The child was present on both days. The medication record indicated the medication was administered at 3:30pm on May 9, 2017 which was outside of the 30 minute time frame for administration. The child's medication record did not indicate a medication error for missed doses and time of administration. Plan of Correction: The Center's procedures will be reviewed with staff. The classrooms' dry erase board will be used to remind staff to administer scheduled medications.
Standard #: 22VAC40-185-560-M Description: Based on direct observations, Staff did not sit with a classroom of children during meal times. Evidence: The LI observed lunch in the 4 year-old classroom. Three staff were present in the classroom serving food to 30 children, staff did not sit with the children during the first 20/25 minutes of the meal. The children at the tables were eating their lunch as different components of the meal were served. Plan of Correction: Staff will be reminded to follow center's procedures. Staff will sit with the children once all foods are served.
