Comments:
The licensing inspector arrived at 1:35pm and departed at 3:30pm to conduct a subsidy health and safety inspection. At the time of the tour there were 55 children, ages infant through five-years, with eight staff. This was a nap time inspection. Children were observed during afternoon snack after nap. The sample size was six staff and four children's records.

Violations:
Standard #: VENDSUB-000-012-B
Description: Based on record review and interview, the vendor failed to ensure that within the first 90 days of employment or subsidy vendor approval; all staff shall complete Virginia Preservice Training for Child Care Staff. Evidence: None of the sixteen staff have completed Virginia Preservice Training.

Plan of Correction: All staff will have this training completed no later than June 7, 2017.

Standard #: VENDSUB-000-012-E-1
Description: Based on record review, the vendor failed to ensure that all staff have current certification in cardiopulmonary resuscitation (CPR) appropriate to the age of children in care. Evidence: Ten of 16 staff lack current CPR certification.

Plan of Correction: All staff will have current CPR certification by June 13, 2017. Half of these staff will obtain CPR on May 13, 2017.

Description: Based on record review, the vendor failed to ensure that all staff have current certification in first aid. Evidence: Ten of 16 staff lack current first aid certification.

Plan of Correction: All staff will have current first aid certification by June 13, 2017. Half of these staff will obtain first aid certification on May 13, 2017.
