An unannounced monitoring inspection was conducted in the program on May 9, 2017. There were 31 children present, 34 children are currently enrolled. The children were in two separate classrooms and each classroom was supervised by 2 staff. Two additional administrative staff were present in the center at the time of the inspection. One staff was observing the program for a portion of the inspection and the other staff was present during the inspection and rode the bus with the children at the end of the program day. The LI observed the children in each classroom and group A. was observed during outdoor play. The Teacher in group B. was observed reading to the children. A discussion was held with the Lead Teacher regarding daily maintenance of the classroom to ensure the floors and area rugs are kept clean. Visible stains were observed on the floor and rugs at the time of the inspection. A sample of 3 children's records was reviewed. A prescribed medication was observed in the Center for one child. Documentation of the Center's annual fire and health inspections was current. Documentation of the practiced emergency drills was reviewed. Reminder: an evacuation drill is due for the month of May Findings of the inspection were reviewed with the Lead Teacher at exit. The inspection was conducted from approximately 11:30am to 2:15pm. Description: Based on direct observations, the Staff did not follow playground procedures to ensure active supervision of the children including positioning of staff in strategic locations, scanning play activities and circulating among the children. Evidence: 1. Upon walking across the grass to the fence and upon arrival to the gate at the playground, both staff were observed sitting on a bench to the side of the playground. There were 14 children present and playing on the playground. After the LI acknowledged the staff by speaking and entered the fenced area, the staff stood up and moved to separate areas of the playground. Children were scattered in separate areas of the playground, several children were in the tunnel. 2. While 2 children were swinging; 3 other children were on the tire swing and other children were near the storage shed, the LI observed both staff on one section of the playground gathered near the fence gate. One staff had been in that location and was talking with several children. The other staff left a group of children on swings and walked to the gate area to retrieve the walking rope. Children were out of sight supervision for several minutes. 3. During the review of injury/accident reports, the LI noted that minor injuries had occurred at the swings with children walking into the path of the swings.

Plan of Correction: The Education Coordinator will have staff review the center's procedures for playground safety, procedures will be monitored ongoing.
