An unannounced Subsidy Health and Safety Inspection was conducted on May 15, 2017 from approximately 4:00 p.m. until 6:00 p.m. Upon entrance there were 24 children and 3 staff present with children. The record review sample consisted of 6 children's records and 3 staff records. Children were observed preparing for and eating snack, working on homework, playing on the computer and board games. The results of the inspection were reviewed with the center director. Areas of non-compliance are noted on the violation notice. Standard #: VENDSUB-000-033-F
Description: Based on observation and vendor interviews, the vendor did not ensure, staff who prepare and serve food to children, or supervise meals, were aware of the food allergies, sensitivities, and dietary restrictions for each child and that staff did not serve prohibited food to a child. Evidence: During the inspection on May 15, 2017, 5 of the 24 children in care had listed food sensitivities/allergy. Two of the children were served items that are listed as sensitive/allergy and or prohibited. Child #2 was served frozen yogurt and diary is listed as a sensitivity/allergy and child #3 was served strawberries and they are listed as a sensitivity/allergy.

Plan of Correction: The director will obtain parents permission to post a list of allergies inside the snack cabinet and review with staff.

Standard #: VENDSUB-000-004-F
Description: Based on observation and vendor interviews, the vendor did not ensure, with parental approval, or kept in a way that is immediately accessible to staff, a current list of all children?s allergies, sensitivities, and dietary restrictions. Evidence: During the inspection on May 15, 2017, there was a list of children with food sensitivities in the staff notebook, however only 2 of the 3 staff were aware there was a list of children with food sensitivities. Parental permission had not been obtained for the posting of the list.

Plan of Correction: The director will provide additional staff training to ensure all staff knows about allergies and periodic reviews. Will also obtain parents permission.

Standard #: VENDSUB-000-012-F
Description: Based on observation and vendor interviews, the vendor did not ensure within the first 90 days of employment or subsidy vendor approval that all staff have completed CPR and first aid training as required by this section within 90 days of the effective date of this agreement. Evidence: During the inspection on May 15, 2017, it was determined 2 of the 3 staff (staff #2 and staff #3) who were required to obtain CPR and first aid training and they did not have the certifications/training. Staff #2 and staff #3 have been working in the center for more than 90 days.

Plan of Correction: The director will ensure they obtain the training.
