Standard #: VENDSUB-000-006-B
Description: Based on review, the licensee did not ensure that each child with a diagnosed food allergy, have a written allergy care plan to include instructions from a physician regarding the food to which the child is allergic to and steps to be taken in the event of a suspected or confirmed allergic reaction. Evidence: The allergy documents for Child #1, Child #2 and Child #3, who all have a diagnosed food allergy, did not include the steps to be taken in the event of a suspected or confirmed allergic reaction.

Plan of Correction: We will use the Safety Sack Emergency Action Plan cards and they will be sent home with food allergy children to be completed by physician.

Description: Based on observation and inspection of the facility, the licensee did not ensure that the written emergency preparedness plan addressed all required components. Evidence: 1. The shelter-in-place procedures did not address accommodations or special requirements for infants, toddlers, and children with special needs to ensure their safety during shelter-in-place procedures. 2. The emergency preparedness plan did not address lockdown procedures to include facility containment and all required components.

Plan of Correction: We will update emergency plan to include shelter in place provisions to accommodate infants, toddlers, and children with special needs and create and post lockdown procedures.
