Comments:
An unannounced monitoring inspection was conducted on 05/11/2016 from 1:30pm - 3:30pm. At the time of entrance there were 55 children, ages infant - five years, in care with eight staff. Children were observed during nap. Three additional staff arrived at the conclusion of nap. Afternoon snack was reviewed. Records were reviewed for six staff and four children. The inspector additionally reviewed medication, written emergency plans, infant/toddler standards to include diapering and hand washing, outdoor play areas, emergency supplies and injury documentation. One violation was observed and is listed on the violation notice. This violation was reviewed with the director at the conclusion of the inspection.

Description: Based on observation, the center failed to ensure that S-hooks where provided may not be open more than the thickness of a penny. Evidence: One swing had two S-hooks that were open more than the thickness of a penny.

Plan of Correction: The S-hooks will be closed up no later than Monday.
