Comments:
An unannounced complaint investigation was begun today in response to an allegation received in our office. Based upon the documentation reviewed, the preponderance of evidence supports the allegation. The complaint is valid for the infant room being out of ratio. Description: Based upon a review of documentation, the infant room was out of ratio today. Evidence: The tracking sheet for the infant room shows that from 7:50 AM - 8:20 AM there was 1 staff person with 5 infants.

Plan of Correction: Staffing patterns will be reevaluated, as well as rosters. Staff will be retrained to ensure documentation is done accurately on the Child Supervision Record (CSR).
