Technical Assistance:
Discussion of consolidating information on tadpole with regards to sensitivities, and dietary restrictions to the posted allergy list.

A SHSI inspection was conducted for the Goddard School located in Ashburn, Virginia. The inspection was done in unison with the Unannounced Renewal Inspection performed on this date as well. Violations:
Standard #: VENDSUB-000-024-D
Description: Based on interview, the center does not currently have a required written procedure in place. Evidence - 1. The Center Director stated (during an interview conducted on the date of inspection - 05/18/2017) that the written procedure that parents shall be notified immediately of any confirmed or suspected allergic reactions, and the ingestion or contact with prohibited food even if a reaction did not occur was not currently in place. 2. It was stated that current policy reflects that the parents are contacted when there is an allergic reaction.

Plan of Correction: This new procedure shall be added to the bottom of our posted allergy list, and shall also be reviewed with all staff.

Description: Based on interview, the evacuation procedures for the center's emergency preparedness plan was not complete. Evidence - 1. The Center Director stated (during an interview conducted on the date of inspection - 05/18/2017) that procedures to address reuniting children with parents or authorized person designated by the parent to pick up the child was not included. 2. Center agreed to expand their emergency preparedness plan to add these procedures.

Plan of Correction: Will include reunification to plan.

Description: Based on interview, the shelter-in-place procedures for the center's emergency preparedness plan was not complete. Evidence - 1. The Center Director stated (during an interview conducted on the date of inspection - 05/18/2017) that procedures to address reuniting children with parents or authorized person designated by the parent to pick up the child was not included. 2. Center agreed to expand their emergency preparedness plan to add these procedures.

Standard #: VENDSUB-000-031-B-3
Description: Based on interview, the center currently does not have procedures in place for a "Lockdown" scenario. Evidence - 1. The Center Director stated (during an interview conducted on the date of inspection - 05/18/2017) that the center does not have a Lockdown procedure. 2. Currently, the only emergency procedures in place and practiced are for evacuations (example: fire) or shelter-in-place (example: tornado).

Plan of Correction: Procedures shall be developed, implemented, and practiced as required.
