Technical Assistance:
Today we discussed the additional requirements for the program's written emergency plan. Please be sure to review the subsidy requirements and ensure that each required element is addressed with regard to Shelter In Place, Evacuation and Lock Down, to include Reunification Plans for each as well as special accommodations for infants, and children with special needs.

Comments:
An unannounced Annual Subsidy Health and Safety inspection was conducted from 11:00 a.m. to 2:30 p.m. on 5/19/17. At the time of entrance, the following adult to child ratios were observed: 10 preschool aged children (3-5 yrs.) with one staff 12 children, two years old, with 2 staff 5 toddlers, ages 16 to 18 months old, with 1 staff 3 infants, 4 to 13 months old, with 1 staff The records sample size consisted of 4 children?s records and 6 staff records. Children were observed playing with bubbles, watching a children's program on television, singing and dancing to music, being read to, creating art, eating lunch, and resting quietly at nap period. Medication administration was not observed. The administrator confirmed that no medication is administered to children in care, by policy. A sampling of 6 minor injury / incident reports was reviewed. Thank you for your time during today's inspection. Please do not hesitate to contact me if I may be of assistance.

Standard #: VENDSUB-000-012-E-1
Description: Based on a review of records and interview, the vendor did not ensure that all staff providing care to children shall have within 30 days of the date of employment or 90 days from subsidy vendor approval, Current certification in cardiopulmonary resuscitation (CPR) appropriate to the age of children in care. Evidence: Six staff records were reviewed. One staff had been employed for only 2 days (staff #6) Staff #1 and Staff #3 had current CPR certification. Records for staff #2, #4 and #5 did not contain documentation of CPR Training. The administrator confirmed that staff #2, #4 and #5 had not yet received CPR Training.

Plan of Correction: The administrator has scheduled and had to cancel several trainings due to staffing needs within the center. Now that the center is fully staffed, the remaining individuals will be scheduled to attend the training one at a time so that coverage may be arranged. It is anticipated that staff will begin training immediately and all staff will have obtained this training within 60 days.

Standard #: VENDSUB-000-031-B-3
Description: Based on a review of the center's written emergency plan and interview, the center's written emergency plan does not contain Lockdown procedures, to include each specified details and a plan for reuniting children with their families. Evidence: The current emergency plan did not specifically address "Lockdown Procedures" to be taken in the event of dangerous perimeter issues or plans for reunification of children with their parents after an emergency.

Plan of Correction: The administrator states that he plans to re-work his Center Emergency Plan to include Lockdown Procedures as well as a specific reunification plan to be implemented after Evacuation, Shelter In Place and Lockdown Emergencies occur. He states that he will have the new plan completed within 30 days.
