Comments:
A complaint investigation was completed at the facility today from 10am to 12:10pm. The inspector interviewed staff and reviewed records in response to an allegation of a forbidden action. Description: Based on staff interviews, the facility failed to ensure that staff do not use punishment by applying unpleasant or harmful substances. Evidence: The administration confirmed the incident. Juice from a lime was rubbed on a child's lower lip in response to a biting incident.

Plan of Correction: The administration was made aware of an incident by a parent in the program at approximately 4:00pm, May 5, 2017. The following actions were taken to immediately address the situation: On Friday, May 5th, at 4:30pm, an internal investigation was opened. Interviews were conducted with each staff member present in the classroom on Friday, May 5th. On Monday, May 8th, two staff members from the classroom were placed on administrative leave until the investigation concluded. During the week of May 15th, all staff members in the Early Childhood Program were re-issued and required to review the current Standards for Licensed Child Day Centers as well as the school's Operation Procedures for Early Childhood. One of the staff members who was placed on administrative pay was welcomed back to the program on Monday, May 15. This staff member, along with other staff in the room at the time of the incident received a written warning that was placed in each person's HR file. On Thursday, May 18th, one staff member was terminated due to the events that took place in the classroom. On Friday, May 19th, staff members in the classroom, where the incident happened, had in-person meetings with the Early Childhood Coordinator to review and discuss any questions related to the school policy and licensing standards regarding forbidden actions. All staff members in the Early Childhood Program will be attending a one hour licensing training session during the week of May 22nd, to ensure they have a full understanding of licensing regulations. All staff will be required to sign a form acknowledging that they received the school handbook, licensing handbook and the school's standard operation procedures information and that they understand the guidelines. This will be completed by Wednesday, May 31st.
