Comments:
An unannounced monitoring inspection was conducted from 8:15am-10:10am. A sample of 3 staff and 4 children's records were reviewed in addition to 4 medications. The center does not provide transportation. Children were observed practicing for the end of the year play. Children were also observed eating snack and infants were playing on the floor with staff.

Violations:
Standard #: 22VAC40-185-270-A
Description: Based on observation the center failed to ensure areas and equipment of the center, inside and outside, were maintained in a safe condition. Evidence: 1. The plastic house on the playground was cracked at the base entryway. The edges were sharp and jagged. 2. In the infant room there was peeling wall paper on the base of the front door.

Plan of Correction: The plastic house will be replaced and the peeling wall paper will be repaired.

Standard #: 22VAC40-185-280-B
Description: Based on observation the center failed to ensure hazardous substances were kept in a locked place using a safe locking method. Evidence: In the 5 year-old and infant room there was a spray bottle of Thieves that were not locked. Thieves warning label says to keep out of reach of children.

Plan of Correction: Staff paced the bottles in locked cabinets during the inspection.
