Comments:
An unannounced SHSI inspection was completed on 5/23/2017. The inspector arrived at 3:00 PM and ending at 5:00 PM. Staff files were reviewed on May 16, 2017 at the main office of the YMCA. There were twenty-two children in care today. The inspector observed the children in the gym area playing several different games and at snack. There were no medications at the site today. The staff files and a sample of children's files were reviewed today.

Violations:
Standard #: VENDSUB-000-012-B
Description: Based on review of three staff records, the center did not ensure that all staff had required trainings. Evidence: 1. Staff # 1 did not have the pre service training as required.

Plan of Correction: The staff will complete the training as soon as possible.

Standard #: VENDSUB-000-031-B-3
Description: Based on review of emergency plan, the center did not ensure that the lock down procedures were added to the plan.

Plan of Correction: The lock down plan will be added with all required components.
