Please contact the licensing inspector promptly upon the return of original, criminal history record checks for Corporation Officers A and B (as identified on renewal study report dated 2/16/17).

Comments:
An announced focused monitoring inspection was conducted from 9:30 a.m.-10:45 a.m. to view background checks for corporation officers. Original central registry checks and completed sworn disclosure statements for Corporation Officers A and B were viewed by the licensing inspector. Documentation reviews and interviews were conducted to verify the submissions of the criminal history record check requests.
