An unannounced monitoring inspection was conducted today March 16, 2017 as a follow-up to the conditional license. The inspection was initiated at 10:45 am and concluded at 12:20 pm. Upon the inspector's arrival eighteen children with three staff were observed. Children were in the elementary school cafeteria having lunch. Staff sat with children and assisted with opening milk. Upon returning to the classroom children had time in centers that consisted of blocks, coloring with markers, pattern cards and house keeping. In house keeping children and staff played with a cash register, pretending to buy groceries and reviewing numbers. Later nap time was observed. All areas of the classroom and cafeteria were inspected. The first aid kit and non-medical emergency supplies were inspected and found complete. All required information was posted on the parent bulletin board. Three staff records and three children's records were reviewed (see violation notice). Medications and medication authorizations were reviewed. Staff on site have current MAT certification, CPR, First aid and daily health observation training. The most recent evacuation drill was documented on 02/24/2017. Two shelter-in-place drills were conducted on 09/06/2016 and 10/20/2016. Today's lunch consisted of chicken wrap with lettuce and tomato, broccoli, refried beans, applesauce and milk. The violations cited at the previous inspection were checked for corrections. Standard #: 22VAC40-185-60-A Description: Based on review of children's records, the facility did not obtain all required information for each staff record. Evidence: The record of child #3 was missing the address for one emergency contact. Plan of Correction: Per the Administrator: we will re-send forms home to get missing information.
Standard #: 22VAC40-191-60-C-1 Description: Based on review of staff records, the facility did not obtain an original criminal history record report within 30 days of employment. Evidence: Staff #2 with a hire date of 02/13/2017, did not have a completed original criminal history check on file exceeding the 30 day allowance. Plan of Correction: Per the administrator: We will obtain background checks within 30 days of hire.
Standard #: 22VAC40-191-60-C-2 Description: Based on review of staff records, the facility did not obtain a central registry finding within 30 days of employment. Evidence: 1. Staff #2 with a hire date of 02/13/2017, did not have a completed central registry finding on file exceeding the 30 day allowance. 2. Staff #3 with a hire date of 12/15/2017, did not have a a completed central registry finding on file exceeding the 30 day allowance. Plan of Correction: Per the Administrator: We will obtain within 30 of hire.
