Comments:
An unannounced complaint investigation was conducted this date. Licensing Inspector met with Owner and Director of center. Staff interviews conducted and observation of physical plant was made. The complaint consisted of allegations of building maintenance, safety and cleanliness. The preponderance of evidence substantiates a VALID finding. Standard #: 22VAC40-185-270-A
Complaint related: No
Description: Based on observation of physical plant and staff interviews, areas of the building are determined unsafe. Evidence: Center Director and staff on site confirmed that there have been mice sighted in the building for the past few months.

Plan of Correction: Center Director is working with a local pest control who has serviced the building in January, April and May 2017. Director will contact another pest control company to determine a more proactive plan to eliminate this issue.

Description: Based on observation of the toddler classroom, a pesticide was found in an area accessible to children. Evidence: Licensing Inspector observed a rodent trapping device that has a toxic pesticide inside which was accessible to children. Licensing Inspector confirmed with the pest control company that the substance is indeed toxic and harmful if ingested.

Plan of Correction: This device was removed from the toddler classroom immediately.

Complaint related: No
Description: Based on director and staff interviews, linens are located in an area that does not ensure they remain clean. Evidence: Staff indicated that there has been an occasion where rodent feces was found on a child's cot.

Plan of Correction: Staff stated that the sheet was removed and the cot was sanitized prior to the child using the cot. Center staff will bag the cot sheets daily to ensure they remain clean and disinfect the cots until the rodent issue is resolved.
