Comments:
An unannounced monitoring inspection was conducted this date. Principal present during the inspection. A total of 79 children in care with 12 staff. Observation of all 8 classrooms was made, two classrooms not in use this date but evaluation of physical plant requirements was made. Children were observed playing on the playground, having show and tell Friday, story time where children were observed interacting with the pop up farm animal book. Greenhouses were found in some classrooms with live plants growing in them. Solar system artwork was on display in the Pre K classroom. Infants were observed being held while feeding and having tummy time. Observation of hand washing prior to eating was observed. Lunch today consisted of chicken nuggets, green beans, watermelon and milk. Staff knowledgeable of which children have food allergies, medications and sensitivities. Staff interviewed to ascertain level of competency of the center's emergency procedures. Recordation organized for staff and children. A total of 5 staff records and 5 children records were reviewed. Four medications on site, all reviewed and determined in compliance. This inspection began at 10am and ended at 1:25pm. One violation found today. A corrective action was provided. Description: Based on child record review, one child record was incomplete. Evidence: Child #2's record did not include documentation of proof of identity.

Plan of Correction: Principal stated that the original birth certificate for this child was verified, however staff did not document the verification. Principal will review the original birth certificate and document the verification.
