The licensing inspector conducted an unannounced complaint inspection on 05/19/17 in response to a complaint that was received in the licensing office on 05/12/17 regarding allegations in the area of forbidden actions. Interviews were conducted with staff , records were reviewed and observations of the program were made relating to the allegation. The information gathered during the investigation supports the allegations, so the complaint is determined to be VALID.

Description: Based on a record review and interviews with staff, the center failed to ensure a child was not roughly handled. Evidence: On 05/08/17 at 4:30pm staff #1 grabbed child #1's arm to usher her in as she resisted coming in from the playground. Thirty minutes after coming inside Staff #2 observed child #1 crying on and off and look fatigued. Staff #2 also stated "her arm looked odd," so she took her to the office. The Director reported child #1 was picked-up by a parent at 5:25pm, taken to the doctor and was diagnosed with nurse maid elbow.

Plan of Correction: We took the appropriate disciplinary action with the staff member involved. The entire staff will be retrained on positive guidance and redirection by 6/5/17.
