Comments:
Thank you for your assistance during this unannounced renewal inspection. This on-site visit was conducted on May 30, 2017 from 10:20 a.m. through 11:30 a.m. and 2:50 p.m. through 6:00p.m. On this date, there were forty-three (43) children in the care of five (5) staff members. Five (5) children's records and five (5) staff records were reviewed and updated. There are one-hundred twenty-seven (127) children enrolled in this program. Areas of non-compliance may be found on the violation notice. Risk scores were discussed. Areas observed were arrival, hand washing/bathroom procedures, snack, and transition, outside play, indoor play and departure. All interactions between staff and children were appropriate and the children seemed comfortable with the daily routine. Standard #: 22VAC40-185-60-A
Description: Based on record review, Child A did not have documentation of the physician's address and phone number. Evidence: Record review for Child A listed the physician's name but did not include the address or the phone number.

Plan of Correction: The parent will be asked to provide the physician's address and phone number.

Standard #: 22VAC40-185-560-K
Description: Based on observation, tables were not sanitized after children ate snack. Evidence: 1.The first group of children sat down to eat snack at the table. They finished and went to the gym to play. 2. The second group of children sat down at the same table to eat snack. The table had not been sanitized after children ate snack.

Plan of Correction: Staff will be reminded that all tables are to be sanitized after meals and snacks.

Standard #: 22VAC40-191-40-D-1-A
Description: Based on record review and conversation with administration, the criminal history check had not been obtained within 30 days for one new board member. Evidence: Review of records on April 18, 2017 confirmed the criminal history check had not been received for the new board member. The new board member began the position on 03/21/2017.

Plan of Correction: The criminal history record check was mailed. The administration will contact the office of background investigations determine the status.

Standard #: 22VAC40-191-40-D-1-C
Description: Based on record review, Staff F did not have documentation of obtaining an updated central registry search. Evidence: Records were reviewed on April 18, 2017. The last documented central registry search was dated 06/10/2013.

Plan of Correction: The central registry search has been mailed. The licensing inspector will be notified once it is received.
