Comments:
An announced initial inspection was conducted on May 31, 2017 from approximately 9:30 AM to 10:30 AM. Three rooms to include a learning room, lounge and kitchen will be used by children. Space and equipment are appropriate for care of school age children.

Standard #: 22VAC40-185-550-B
Description: Based on review, facility did not ensure the emergency preparedness plan has required components. Evidence: On May 31, 2017, the emergency preparedness plan did not contain the following: emergency officer and backup officer to include a 24-hour contact phone number for each, assembly points out side for evacuation, sounding of alarms for shelter-in-place, securing parent contact information and special healthcare supplies.

Plan of Correction: Director will add all the specific information into the plan today.
