Comments:
The licensing inspector conducted an unannounced monitoring inspection Wednesday, May 31, 2017 from 12:03pm to 2:47pm. The children were observed finishing lunch and getting ready for rest time. Lunch consisted of grilled cheese, tomato & cucumber salad, peaches, and milk. The lunch menu was posted. All classrooms and playgrounds were inspected today as well as the facility?s mini buses. Four children?s records and two employee records were reviewed during this inspection. Medication is being administered and was reviewed. The center's first aid kit and emergency supplies were inspected. Last emergency drill: 05/25/2017 Last shelter-in-place drills: 10/18/2016 Last fire inspection: 09/12/2016 Last health inspection: 03/03/20176 Total enrollment today: 97 Today, the following child to staff ratios were observed: Infants ?8:2 Pre-Toddlers ?8:2 Toddlers ?10:2 Twos ? 23:3 Threes ?20:2 Pre-K ?28:3 School Age ?not present The violations from the previous inspections were checked for corrections. Standard #: 22VAC40-185-130-A
Description: Based on a review of children's records on 05/31/2017, the center did not ensure that two of the four records reviewed contained documentation that the immunizations required by the State Board of Health were received before the child can attend the center. EVIDENCE: 1. The immunizations in the record of Child #1, whose start date at the center was 06/16/2015, are dated 06/19/2015. 2. The immunizations in the record of Child #3, whose start date at the center was 08/01/2016, are dated 08/19/2016.

Plan of Correction: Will ensure immunization records are obtained on or before the start date.

Standard #: 22VAC40-185-130-B
Description: Based on a review of children's records on 05/31/2017, the center did not ensure that one of the four records reviewed contained documentation of additional immunizations once every six months for children under the age of two years. EVIDENCE: The most recent immunizations in the record of Child #3, who is 12 months old, are dated 08/19/2016.

Plan of Correction: Emailed mom today to bring in by Friday.

Standard #: 22VAC40-185-330-B
Description: Based on observation on 05/31/2017, the center did not ensure that where playground equipment is provided, resilient surfacing shall comply with minimum safety standards. EVIDENCE: The depth of the mulch on the pre-k/ school age playground with the climbing tunnel is less than six inches.

Plan of Correction: Until we can get new mulch, we will fill in mulch from the hill to go around the equipment. Will get quotes for new mulch this afternoon.

Standard #: 22VAC40-185-510-N
Description: Based on review of medication on 05/31/2017, the center did not ensure that when an authorization for medication expires, the medication was picked up within 14 days, the parent renewed the authorization, or if the medication is not picked up by the parent within 14 days it will be disposed of by the center. EVIDENCE: The authorization for prescription medication belonging to Child #5 expired on 04/14/2017.

Plan of Correction: Emailed mom to pick up medication today or bring in a new form.

Standard #: 22VAC40-185-520-A
Description: Based on observation on 05/31/2017, the center did not ensure that over-thecounter skin products shall not be kept beyond the expiration date of the product. EVIDENCE: 1. An expired tube of diaper cream belonging to Child #3 was observed in the infant room. The cream expired 03/2017. 2. An expired bottle of sunscreen belonging to Child #2 was observed in the pre-k room. The sunscreen expired 11/2015.

Plan of Correction: Will discuss at our next staff meeting. Teachers will regularly check expiration dates to ensure everything is updated.
