Comments:
An unannounced health and safety inspection was conducted from 7:15 am - 9:45 am. There were 26 children in care being supervised by four staff members. The children were observed participating in table top activities, and structured center play. Lunch served today is fish on wheat bun, peas, peaches, and milk. Ten children's records were reviewed and in compliance. Training documentation was reviewed for 15 staff members for CPR, First Aid, and Virginia Preservice Training. The most recent emergency escape drill was conducted 4/20/2017 and the most recent shelter in place drill was conducted 2/24/2017. Standard #: VENDSUB-000-012-B
Description: Based on observation of training documentation it was determined that not all staff completed the Virginia Preservice Training within 90 days of vendor agreement. Evidence: Staff B, Staff C, and Staff D did not have documentation of completing the Preservice Training.

Plan of Correction: Staff B, C, and D will complete the Virginia Preservice Training.

Standard #: VENDSUB-000-012-F
Description: Based on observation of staff training documentation it was determined that not all staff completed CPR and First Aid Training within 90 days of the vendor agreement. Evidence: Staff A, Staff C, Staff D, and Staff F did not have documentation of completing CPR and First Aid Training.

Plan of Correction: Staff A, C, D, and F are scheduled to attend training on June 10th, 2017.
